FOR ALL STAR AFFIDAVIT MEETING
WEDNESDAY JULY 2, 2008
HOLIDAY INN
4520 - 76 AVE
The President and Player Agent
will be at the meeting to check your forms and sign them.
All teams will need to have
the 3 copies of the affidavit form completed along with copies of birth
certificates. The proof of residency will be required for Senior, Major and
Junior teams. All teams will require the Medical and Travel forms along with
the boundary map completed.
Meeting Times
6:30 to 7:30
pm Minor AA
7:45 to 8:30
pm Minor, Major AA and Junior AA
8:45 to 9:30 pm
Major, Junior and Senior
A short meeting will be held
to go over schedules, guidelines, tournament rules and coaches questions.
Umpires will be in attendance to answer any rules questions.
After the meeting the
affidavits will be checked and signed. Before coaches leave the District
Administrator must sign the affidavits. |
